Need to see a counselor?

Submitting a referral for a CCMS student is simple! This one form will allow a parent, teacher, or student to submit a referral for a student to be seen. 

As always, if you or someone else is in immediate danger then please do NOT use the referral form. Get this information to a trusted adult immediately. Also, you can call the National Suicide Hotline @ 1-800-273-TALK (8255) or you can text them @ LISTEN (741741).

We strive to teach every CCMS student that they deserve mental wellness while giving them the support and education to pursue it.

Mental Wellness & Suicide Prevention:

 We have chosen to implement a mental health and depression awareness program called We All Have Mental Health for our 6th grade students, Level II: Depression Awareness for our 7th grade students, and Level III: Depression Education & Suicide Awareness for our 8th grade students. These programs were developed by a highly respected mental health education and advocacy organization, Erika’s Lighthouse: A Beacon of Hope for Adolescent Depression. For more information, you may visit: https://erikaslighthouse.org/ We will go into all classrooms in September and again in January to encourage and remind CCMS that every student deserves good mental health while giving them the information, resources, and support they need to seek help for themselves or a friend.
